What is obesity?

Obesity is when the body stores excess calories as fat, leading to weight gain.(1) It’s defined as a BMI of 30 or above. Your BMI is basically a measure of whether you are a healthy weight for your height. It’s calculated by dividing an adult’s weight in kilograms by their height in metres squared.(2) There’s a common misunderstanding that obesity or a high BMI is simply caused by factors within someone’s control, such as eating unhealthily and not exercising enough. This mistaken belief can cause stigma, and discrimination.(3,4)

30% of adults aged 16 and over were living with obesity, and 66% were either overweight or living with obesity.(5) Living with overweight and obesity increases your risk of developing many other serious health conditions, including:

  • type 2 diabetes
  • heart disease
  • stroke
  • high blood pressure
  • some cancers, such as breast cancer, womb cancer and bowel cancer
  • osteoarthritis
  • fatty liver disease(2,6)

Living with obesity is likely to lower your life expectancy by between 2 and 10 years.(2)

Obesity is not a choice, and each person’s journey to lose weight is unique. Some people find that a weight-loss program may not always lead to weight reduction due to the body’s adaptive mechanisms to resist weight loss.(7,8) Empathy is important to effectively support someone’s weight loss journey.

Why BMI Matters

Why BMI matters

The National Institute for Health and Care Excellence (NICE) recommends tirzepatide (Mounjaro®) for the management of obesity for certain patients. Tirzepatide (Mounjaro®) is available on the NHS to those with the highest clinical need. From March 2025, people living with obesity who have been diagnosed with 4 of the weight-related health problems listed below and a BMI of 40 or more (adjusted for ethnicity) will be eligible to access tirzepatide (Mounjaro®) through primary care settings.

  • dyslipidaemia (abnormal fat levels in the blood)
  • hypertension (high blood pressure)
  • obstructive sleep apnoea (when your breathing stops and starts while you sleep)
  • cardiovascular disease (heart and blood vessel disease)
  • type 2 diabetes mellitus

That’s because BMI is an important clinical measure of whether weight loss injections are right for you. It assesses whether you will gain sufficient benefit from the treatment to justify any potential side effects.

What is a healthy diet?

A healthy diet is everything that you eat and drink. It means eating mostly fruits and vegetables, wholegrains, like wholewheat pasta, oats and wholemeal bread, and healthy sources of protein such as fish, pulses or chicken.

It also means cutting down on processed and red meat, alcohol, and foods that are high in fat, sugar and salt. In the UK, the Eatwell Guide shows the different types of foods and drinks to consume, and in what proportions to have a healthy, balanced diet.(1,2) It’s not about following strict rules. Think of it as a guide to what you should eat on an everyday basis. You can still eat the things that you love, whether that’s crisps, chocolate or cakes just more occasionally and in smaller portions.

Try to follow the recommended daily caloric intake for adults including all food and all drink. For women that’s 2000 kcal and men it’s 2500 kcal.(1,2)

It’s not just about what you eat. Physical exercise is important for maintaining a healthy weight too and has health benefits beyond weight loss.(3) The NHS recommends:

  • At least 150 minutes of moderate intensity activity per week.
  • Spreading exercise evenly over 4-5 days per week or every day.
  • Strengthening activities that work all major muscle groups on at least 2 days per week.
  • Reducing time spent sitting and lying down.
